Scheduling

One of my ways of reducing stress is by streamlining scheduling to meet your individual needs.

Appointments may be made for daytime, evening or weekend hours.  When appropriate, consultations by telephone or video calls on Skype are also available.

Cancellation

I am a preferred provider for Premera and Regence and also accept Lifewise, Uniform insurance plans. I am an out-of-network provider for many other insurance companies. Please check with your insurance
company to find out what your benefit and co-pay is.

 

Payment or co-payment is due at the time of service. My billing company, Northwest Clinical Billing will send you a monthly statement for your records.

My cancellation policy is as follows:


I require 24 hours notice if you need to cancel your appointment. There are exceptions to this policy, i.e. if you or your child is ill.

 

For telephone consultation exceeding five minutes, I charge on a pro-rated
basis. Please note that telephone consultations are not billable to insurance.
